Business unit contact form

Enables visitors to provide information about themselves, send a message and submit the form. Form information will be sent to an email address specified when the component is configured.

Overview

Form fields are fixed but their visibility is customizable. To create a form with different form fields, consider using a Marketo form. Business Unit contact forms are only available when using a Business Unit Contact page template.

Configuration settings

  • Form Title: Title that appears above the form
  • Form Description: Text that appears to the left of the form
  • Send to Email Address: Email address that the form will send to if no topics are set
Submission behavior settings
  • Form Thank You Message*: Inline message that will appear when the form is submitted
  • Page Redirect: Page the visitor will be redirected to if the "Page redirect" behavior is selected
Disclaimer settings
  • Add Form Disclaimer Checkbox?: Displays a checkbox that the visitor can opt to select
  • Form Disclaimer Checkbox Required?: Requires the checkbox to be selected before the form can be submitted
  • Form Disclaimer: Text that will for the disclaimer

Topic list settings

Authors can configure optional topics that will appear as checkboxes to the left of the form. For each topic, the following settings are available:

  • Label*: Topic name (e.g. Send me a brochure)
  • Description: Optional description
  • Email*: Email that the form will be sent to to fullfill the request
