Model release form 
I hereby give to Howmedica Osteonics, its legal representatives and those acting under Howmedica Osteonics’ permission and authority, the irrevocable and unrestricted right to use and publish video footage and/or photographic images of me or any excerpts from such video footage and/or photographic images (a) for editorial, advertising, trade or any other legal purpose; (b) in any manner and medium; (c) at such time as Howmedica Osteonics may determine; (d) to alter the same without restriction; and (e) to own the copyright to the same. I acknowledge that the permission and authority I have granted to Howmedica Osteonics here under is on a voluntary basis, and that it is neither required as a condition of my employment nor in any way related to my position with Howmedica Osteonics. 

I am of legal age and have the right to contract in my own name. I have read this authorization and fully understand it. I also understand that nothing in here obligates Howmedica Osteonics to use the video footage and/or photographic images of me or any excerpts from same for any purpose. 

I hereby release Howmedica Osteonics, its legal representatives and assigns from all claims and liability relating to the use by Howmedica Osteonics of such video footage and/or photographic images of me.
